Services Eligibility

Our services are available to individuals who need support to live safely and independently. Eligibility is based on residency, Medical Assistance (MA) qualification, and an assessed need for services through state or county programs.

Basic support services

These services offer the assistance, supervision, and care needed to ensure an individual’s health, safety, and ability to live independently in their home and community.

ILCS is designed for individuals who need reminders, cues, intermittent or moderate supervision, or physical assistance to continue living safely in their own homes.

This service provides on-call counseling, problem-solving, and immediate response for health or personal emergencies that occur in the individual’s home, available 24/7.

Adult Companion services provide non-medical care, supervision, and social interaction for adults age 18 or older. Homemaker services include light household cleaning, home management, and incidental assistance with activities of daily living.

Night Supervision provides overnight monitoring and assistance by awake staff in the individual’s home. Respite Care offers short-term support when a family member or primary caregiver needs temporary relief or is unavailable.

These are services that provide the level of assistance, supervision and care that is necessary to ensure the health and safety of the person. 

  • Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes.
  • 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person’s home due to a health/personal emergency.
  • Adult Companion: Non-medical care, supervision and socialization to a person age 18 or older.
  • Homemaker: Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.
  • Night supervision: Provides overnight assistance and monitoring by an awake staff in the person’s home.
  • Respite care: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

Intensive support services

  • Semi-independent living skills: Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community.
  • Individualized home supports: Provide the support in the areas of community participation, health, safety & wellness, household management, and adaptive skills.
  • Individualized home supports with training: Provide the support and training in the areas of community participation, health, safety & wellness, household management, and adaptive skills. The training is designed to help the person develop the skills they need to live independently.
  • Individualized home supports with family training: Provide the support and training to both the person with a disability and their family. The training is designed to help the family learn how to support the person in their home and community.

These services support adults with developmental disabilities or related conditions by helping them develop the skills needed to live successfully and independently in the community.

Individualized Home Supports with Training includes hands-on instruction in addition to support. The training helps individuals learn and build essential life skills needed for greater independence.

Individualized Home Supports Without Training – Izzy Homecare

Izzy Homecare provides individualized home support services without training for adults and children living in their own homes. These services focus on assistance, supervision, and daily support delivered by trained staff. Our caregivers offer direct supervision, reminders, cues, guidance, help with daily living activities, and support with coordinating and participating in community-based activities. Services may be provided in the individual’s home or within the community, based on their needs.

Individualized Home Supports Without Training were introduced through the State of Minnesota’s Waiver Reimagine initiative, combining the former Personal Support and Adult Companion services into a single, streamlined service.
